Eufaula Head Start is accepting applications for the 2012 –13 school year.
Applications can be completed at the Head Start Center at 525 School St. Monday through Friday from 7 a.m. to 3 p.m.
The following documentation is needed to complete an application:
» Child’s birth certificate
» Proof of total 2011 household income
» Proof of medical insurance
» Up-to-date immunization record (Blue card)
The applying child’s parent or legal guardian must complete the application. For more information, call 687-2796.
